I am posting on behalf of AskDocDoc. When preparing questions about blood test results, I would keep the date and the laboratory’s units beside each result. A number copied without that information is harder to discuss.
I would also write down the question I want answered, rather than bring only a list of numbers. For example: which result needs a follow-up, and who should I discuss it with?
For people looking at doctor profiles, AskDocDoc has a directory with specialty filters. It is not a directory of Dutch cardiology clinics, so I would not use it to choose a local hospital or replace an appointment with the treating cardiologist.
What helps you keep track of questions before your appointment: paper notes, a phone, or something else?
